Job Description:
The Social Worker – Disease Management collaborates with the clinical team to assess, plan, implement, coordinate, monitor and evaluate the beneficiary's health and psychosocial needs. The focus is on empowering beneficiaries to successfully manager their medical, behavioral and psychosocial needs related to their chronic conditions.

The social worker partners with the interdisciplinary care team to identify and implement appropriate care to prevent complications related to chronic conditions and minimize the risk of exacerbations using evidence-based, condition-specific tools and support. This role involves contributing to care planning in collaboration with clinicians, providers, patients and families, as appropriate, while evaluating the effectiveness and outcomes to improve health status.



Responsibilities:

  • Assess beneficiaries to identify medical, physical and psychosocial needs, supporting their overall goals of care.
  • Collaborate with the clinical team to contribute to the whole-person care by incorporating behavioral and psychosocial assessments, using evidence-based guidelines and clinical reasoning.
  • Provide education and coaching support specific to the beneficiary’s behavioral and psychosocial needs to prevent complications and support chronic condition management.
  • Ensure appropriate utilization and access to medical and behavioral resources while maintaining optimal standards of care.
  • Identify and facilitate resources related to health disparities through Social Determinants of Health assessments.
  • Participate in developing measurable outcomes and ensure metrics are in place to monitor the impact of programs and services on the health status of beneficiaries.
  • Assesses high-risk factors, acuity and need for other services and resources.
  • Facilitate beneficiary transfers between regions by collaborating with the military liaison to ensure smooth transitions with minimal disruption.
  • Adherent to URAC standards, company policies, contractual requirements and nationally recognized practice.

Requirements:
  • Current, unrestricted LCSW license to work independently within the scope of practice within state of residence (BSW acceptable)
  • U.S. citizenship
  • Minimum of two years of experience in case management, care coordination or discharge planning in an acute, ambulatory or specialty disease management program settings within a healthcare system or managed care organization
  • Availability to work in Central and Mountain Standard Time zones, regardless of location

Pay Range:

The anticipated pay range for this position is $35-$43 hourly.

The final pay for this position is determined based on several factors including but not limited to, relevant experience, skills and certifications, education and training, geographic location, market conditions, and internal equity.
